To help your visit to Fulton County Vision Services be as smooth as possible, take into consideration the following advice.
Please plan on arriving a few minutes early so that you can complete a short General Information form.
If you are covered under VSP Insurance, please only fill out Form 2.
If you normally wear prescription glasses or sunglasses, please bring them in. Also, if you are being seen for a contact lens exam, please wear your contact lenses to the appointment. And if you are a new patient, bring the container your contact lenses came in if possible.
Bring your vision and/or health insurance cards with you and a list of all medications you are currently taking.
Depending on your exam, the Doctor may need to dilate your eyes. This dilation may last for several hours, so you may want to bring a pair of sunglasses to wear home, and in most cases you will be able to drive home yourself. We will provide sunglasses for you if needed.
Our Financial and Insurance Policy
At Fulton County Vision Services, we require payment for services at the time of your appointment. In most cases, we will bill your vision or health insurance carrier as payment minus any co-payments that may be required. If you do not have insurance coverage or we are not a provider for your health plan, you will be expected to pay for your services at the time of your visit and at least half down payment on any eyewear or contact lenses ordered by our office. The full balance of your bill would be due at the time of dispensing.
Since all insurance plans are different, it will be your responsibility to check with your personal insurance carrier to determine if you are eligible for eyecare coverage before your appointment at our office. The following insurance plans are generally accepted:
• VSP (Vision Service Plan)
• Central States
• Paramount Advantage
• Sterling Option 1
• Super Med Plus
• Superior Vision
Please bring your current insurance card with you. Our staff has experience working with many different plans and will be happy to assist you with any questions you may have.
